Our Comprehensive Assessment

At Spectrum, we provide an individualized care plan specific to the needs of our patients. We begin this process by providing a free, in-home assessment by one of our experienced RN case managers. Next, we collaborate with other health care professionals who are currently providing care to the patient. Finally, we obtain input from the patient and involved family members to ensure an all-encompassing care plan.
